Database errors when I add banner to header

Hi, I desperately hope someone can help me. Ever since I moved from 1and1 to ASO hosting, I've not been able to place banner ads in my VB header. It works fine when by itself:

http://www.warwickshireonline.com/extra/header2.php

But as soon as I try to impliment it into the header here (www.warwickshireonline.com), I get database problems.

Please advise, it worked fine with my old host and this is really frustrating me.
