As in, what ones do you think every forum should have because they're that great, and which do you think ruin any forums they're added to?
For me, I find the best modifications the vB Credits Point system ibparcade (or whatever it's called), Photoplog and quite a few others. My least favourite ones are right click prevention scripts, scrolling status bars and huge stats lists at the bottom of forums.

IMO,
anything that annoys, popsup, limits me, warns me, or alerts me to something like i'm an idiot should never be installed.
This includes welcome headers, thats stupid ie-like activex register bar, and so on.
my fav mods are:
vbseo / vbseo sitemap generator
Who tagged?
paulm's databackup
dismounted's ebay rewrite

The Arcade and Thanks features are my faves. I don't install mods I don't care for hence why I can't say my least faves.

You could still say the ones you would least want to install and why.

vBSEO and vBadvanced CMPS.
Done great things with the two software and can't seem to start a forum/site without them.

My favorite mod -ever- released for vBulletin is the Itemshop v5 for vB2.
Second favorite is the Welcome Panel hack by trilObyte, then the Shoutbox by p0s3id0n. Both also for vB2.

Well, since the moderators are allowing us to slam some modifications (for some reason), I'll have to say my least favorite would have to be vBadvanced. It really serves no purpose, and it is extremely over-used.
My all time favorite would have to be.. GAL. I used it in a way to not annoy my members, but actually help them know what the hell everyone was talking about, and it worked out great.

My least favorite? hmmmmmmm.... I don't have one really, since I don't install anything I don't like or need.

This includes welcome headers, thats stupid ie-like activex register bar, and so on. If there is one thing that stops me from registering on a forum, it is that modification!

Yep, that isn't the worst modification, but it's one of the worst. The worst register one is the one that's like flash or something that scrolls down the page with you, that pisses me off so much. I refuse to join a board that has that.
------------------------------------------------------
Worst Modification
------------------------------------------------------
vB Advanced - aka make everyone's forum have a fake front page that looks the same. I don't care about the options, it's overdone and they all look the same.
------------------------------------------------------
Best Modification
------------------------------------------------------
Passive Vid Not BB Code - simply the best video integration into forum mod there is. Nothing compares IMO.
------------------------------------------------------
Can't live without spoiler and stack rank products, the latter being a fix to the vb code.

You mean by the flash thing the scrolling Internet Explorer security alert style bar found on some forums? Yeah, that's incredibly annoying. Nothing however compares to 'Right Click prevention' scripts though, probably the worst joke on the internet.
As for vB Advanced, I don't really mind it simply because I don't pay any attention to it, and just click through to the forum listing anyway.

My favourite modification is the procrastinating hack that allows you to switch your forum offline by simply turning your board offline by thinking keywords like "offline" " taking a break" "au revoir" so you can take a break from having a break. That and maybe the "bump thread hack" which I suggested nick to code.
My least favourite modification are the ones you have to install.
