Add user note on ban and lift ban

This hack is originally from here:
http://www.vbulletin.org/forum/showthread.php?t=59540 I've ported it to vB 3.5 because it's a necessity for our forum. Very simply, this adds an additional required textarea to the ban and lift ban forms. Every time you ban or lift a ban on someone you enter a "private" reason which will be added as a user note for the user, effectively creating a ban history log that you can refer back to by looking at anyone's user notes. Note, this is unlike the vB 3.5 built-in ban reason, which is publicly displayed to a banned user. That built-in reason is still there for you to use, this is a private reason for you and your moderators to look back on. So this means we have to fill out two ban reason fields ?
Or does it mean we fill out the same field and have it display the ban reason to the banned user and logging the reason to usernotes ? No members have liked this post.

I've attached some example screenshots to clarify.
There are two reasons. One, the reason built-in to vB that will display to the user when they try to visit the forum while banned. The other (which is what this hack is) is a private reason that gets added as a user note so you and your mods have a history.

No, sorry. I would have if I could have.
There are no hooks in this section, plus, it requires changing the form, which isn't in a template so even with hooks you'd still need to edit the source.

Hey anyone know if this can get a extra conversion to make a thread in "x" forum with the thread title of the banned user, then the reason would be posted in the thread, same as the usernote? Or is this covered in things like AwS?
