Create a form that submits to the databse

I want to create a form that submits to the database and then updates itself based on what was submitted to the database similiar to the Administrator Notes function in the Admin CP.

Are there any good tutorials about this that someone can point me to? Ideally I want to put it at the top of my staff forum and the staff members can update this with assignments they are working on etc.
