Question Contest Page - Football Contest - Click for Request

I own a college football website. I want to have a contest page where the user selects the winner of pre-determined games. I want them to only be able to submit an entry once, and I would like for there to be a master input where I can put in the results and it automatically tabulates the winner. I'm not sure how difficult this is, but would really like to have this set up by August. If anybody can help me with this, I'd appreciate it (and can give more detailed information as you need it).

If you require a fee, within reason, I can pay...but I just need this to be functional and reliable...it doesn't have to be pretty.

Here's a little more detail now.

The page should basically be set up in 3 columns.
The two teams should be be put in the two outside columns with a radio button (or check mark) where the user can only select one team. The middle column (between the two teams) can be small in width and will be where I indicate the spread (this is what the users will be selecting the winner based on).

I would like for the last 2 games to have a tie-breaker field of writable text where they will put their score guess for each team. After they submit, I would like for it to save it and then after I update the master sheet (or whatever it is) with the correct winners, it calculates which user has the highest percentage right and displays the top ?? in a chart.

Sound difficult? I can't imagine it will be too hard, but I'm not smart enough to do it...can anybody help me?

I suggest you repost it in Service Request forum if you are willing to pay. Some coders only look there.

Make sure you read the stickied posts in the top first.
