Forms Posted to User Profile

Hey Guys,

I would like to add a feature to the User Profile on my forums.

Basically, a user fills out a form, and the information is posted to a database. That database can be seen by admins, and also the user who posted the form, but no other user.

The user will be able to see what they have posted in the past (as the form I have in mind is a Roll Call).

Any way of doing this? Or is there a similar mod out there that i could adjust?
